Disclosure: Foundry PC wrote this comparison and is one of the options compared. Criteria: who owns the PC, whether pricing is published, whether you keep your own entity, and which states are covered. Competitor details come from each vendor's public website as of the review date above.

Foundry PC is not a law firm. General information only, not legal advice. Guardian Medical Direction and GuardianMD are names of their owner, used here nominatively so you can tell the offers apart.

Foundry vs Guardian Medical Direction compares an entity-formation service with a physician-oversight service. Foundry PC forms your MSO-PC for the states you operate ($15,000) and matches a friendly PC owner, the licensed physician who owns that Professional Corporation ($3,000/mo). GuardianMD's public homepage describes collaborating and supervising physicians for NPs and PAs, medical directors for clinics, franchises, and multi-location groups, direct prescribing, credentialing and contracting, telemedicine clearance exams, and a platform for protocols, chart reviews, and documentation. The same homepage lists PC/PLLC creation and ownership and PC/PLLC and MSO structuring support.

Source for GuardianMD's column: the guardianmedicaldirection.com homepage, reviewed 19 October 2026. It advertises no setup fees, a typical physician matching time of 7 days or less, and licensed physicians in all 50 states. It does not publish a monthly price. Foundry publishes $15,000 formation and $3,000/mo matching. Confirm GuardianMD's current offer and price on its own site. Nothing here is a review of GuardianMD's clinical quality. Which one fits your company

If you run an NP-led practice or a med spa and the missing piece is a collaborating physician or a medical director with chart review, an oversight vendor is built for that. GuardianMD markets that service across many clinic types, including aesthetics, IV therapy, weight loss, and primary care.

If you are a non-physician founder and the missing piece is the Professional Corporation itself, the job is different. You need a licensed physician on the shareholder ledger, articles and bylaws that fit each patient state, an MSA at fair market value, and the claim TIN on the PC. That is what Foundry sells. A medical director title on your website does not make that physician the owner. See the med spa hub for how the roles split in aesthetics.

Many companies need both: a PC owner for the entity, and collaborating physicians or a medical director for daily clinical work. Ask every vendor, Foundry included, the same three questions. Who is the shareholder of record of my PC? Which states is that physician licensed in? How is the management fee set? State rules differ. South Dakota requires licensed officers and directors as well as shareholders, West Virginia requires a Board of Medicine certificate before a corporation practices, and Utah requires a domestic professional entity.
