Operator deep page (statute cites, PC vs MSO table, Foundry formation CTA): Oklahoma CPOM on the CPOM 50-state guide. Pair it with CPOM compliance and MSO-PC formation ($15,000). A friendly PC owner is $3,000/mo.
Oklahoma is Limited, not a no-rules state. Official 18 O.S. § 803 defines a professional entity as a domestic or qualified foreign corporation, limited partnership, or LLC formed to render professional service, including physicians licensed under Title 59. Official 18 O.S. § 809, as quoted in 2025 OK AG 16, still requires licensed (or otherwise permitted) owners of a domestic professional entity. 59 O.S. § 492 licenses who may practice allopathic medicine.
Do not stretch SB 620
Enrolled SB 620 (2024) added a foreign-entity clause: an owner of a qualified foreign professional entity need not be licensed if that owner is not rendering professional services in Oklahoma. Domestic professional entities still need licensed owners. It is not a digital-health startup waiver. Oklahoma City diligence still wants a physician-owned PC plus an FMV MSA.
What Oklahoma City founders should do
- Form an Oklahoma professional corporation owned by an Oklahoma-licensed MD or DO.
- Keep the § 809 owner file with the Secretary of State and the Board of Medical Licensure and Supervision.
- Pay the MSO a fixed or FMV-supported admin fee, not a percentage of collections.
- If you also treat Kansas, Texas, Arkansas, Missouri, or Colorado patients, open those deep pages. Oklahoma’s Limited label does not control them.
